Physical Therapy Treatment & Modalities - ₱ 80.00

Ultrasound, TENS/ES, Infrared Ray, Paraffin Wax Bath, Hot Moist Pack, Icing, Consultation Physical Examination by Health Officer (For each additional copy of medical certificate) - ₱ 30.00

Seal - ₱ 50.00
